Silverado At Home’s Life Care Managers (LCMs) play a vital role within our service line, expanding our reach and personalizing care for every client. Silverado At Home Bay Area’s LCM is Tammy Won, a licensed nurse who is able to bring this personalized care and nursing oversight to all of our clients. Along with assisting in the creation of personalized care plans, behavior mapping and monthly follow-ups with the family and client, she also is a tremendous help with our caregivers.

Applying her nursing background, Tammy trains our caregivers in 
active daily living assistance skills and presents monthly sessions that ensure our caregivers bring the Silverado philosophy and quality of care everywhere they go. Enough can’t be said of what Tammy brings to our At Home office, and a big thank you needs to go out to Tammy, all of our Life Care Managers and nurses for the great work they do.
